Online Golf Tournament Sign Up & Payment
This page will allow participants to either:
A) Register and Pay by Mail Download a "Foursome Form" to sign up from 1-4 golfers or diners. Please click the downloadable pdf file below and fill out the form with your name and contact information. B) Register and Pay Online
i) Register now individually or up to a Foursome and Pay Now (PayPal) using a credit card to pay for each Golf + Dinner Ticket ($175 per person + service fee) OR ii) Register and pay for a Dinner Only Ticket and/or up to two Dinner Only Tickets ($75 per person + service fee). OR iii) Pay for a Corporate Sponsorship (Bronze, Silver, Gold) "Online Registration Form" Instructions: (Note: each Golfer or Dinner Guest must fill out their form individually and click "Submit" before registering the next Participant) STEP 1: Fill out your first and last name. STEP 2: Fill out "Foursome Name/Dinner Guest Group" Box. A) If you are a GOLFER: - Are you with another golfer(s) but registering separately? Then, choose a Foursome Name that is either... a) The last names and first initial of each player in your group (eg. Smith J., Cohen H., etc.) b) The name of the company sponsoring your Foursome (ABC Inc.) c) The name of your Foursome "Captain" ie. the key individual organizing your group - Not part of a Group or Foursome? Then, please indicate "Individual" and you will be put with a group. No worries, everyone is friendly! B) If you are coming for DINNER ONLY: a) Coming for dinner alone? Then, write "INDIVIDUAL". b) Do you have a preference as to where you will be seated? Yes: Then, indicate the names of those you would like to be seated with and we will do our best to accommodate your request. No: Then, indicate "INDIVIDUAL - NO SEATING PREFERENCE" c) Are you registering as a COUPLE for "Dinner Only"? Then, write both Last names and First Names in this field. STEP 3: All information in 'address field' must be complete in order to issue your tax receipt. STEP 4: Email address - Include up to two addresses (work, home). STEP 5: Indicate Home and Cell numbers (eg. H: 514-555-1212, C: 514-555-3313) STEP 6: Check the box for either "Golf and Dinner" or "Dinner Only". STEP 7: If you are coming for Dinner and would like Bus Transportation for $10, please check the box "Yes" under Bus Service. Payment for the bus can be made at Holy Family parish on the day of the event. STEP 8: Check off the Tax Receipt Box ONLY if the tax receipt should be mailed to you. A tax receipt of $70/golfer and $25/diner will be issued IMPORTANT: If you are registering but a third party should receive the tax receipt: a) indicate to whom it should be issued via email by CLICKING on the "HERE" button below to send an email to the tournament administration, OR b) include a note during the PayPal transaction in the "Add Special instructions to the Seller" box (eg. Issue tax receipt under Jack Smith) STEP 9: Click on "Submit". Your form will immediately be sent to the Golf Committee for processing. STEP 10: Click on "Buy Now" and follow PayPal instructions. Fill in all fields to complete the secure payment using any major credit card. You do not need to have a PayPal account to pay using this option. *A small fee is applied for this convenience to offset the service charges applied by PayPal. STEP 11: Repeat STEP 1-10 to register another Golfer or Diner. STEP 12: Wait for a confirmation that you are officially registered. Please allow 3-5 days. If you have not received any e-mail confirmation within that time frame, please click the Contact Us "HERE" link below and send us an email. Special Requests: * If you have any special menu requests such as substituting steak for pasta or chicken, or Vegetarian, you can send us a message using the Contact Us "Here" button below OR indicate this preference when you complete the PayPal transaction ie. In the box "Add special instructions to the Seller" make your request and we will do our best to accommodate you. If you have any comments, questions or concerns. Please contact us by clicking HERE.
